When this helps
Paper forms and one-off documents make it difficult to know what was requested, what a client submitted, and which version belongs to an appointment.
How Yuna helps
Create a form with Yuna
Describe your intake or consent form in plain language and Yuna can build a draft with questions and signature fields. You can also start from a template or build it yourself. Review and edit the wording before use.
Set up automatic delivery if needed
Link a form to a service and choose when it should be sent, including booking, confirmation, or appointment completion. For a walk-in, you can also use kiosk mode in the iPhone or iPad app.
Collect a signature online or in person
Send a time-limited link for the client to complete remotely, or open kiosk mode in the iPhone or iPad app and hand them your device for in-person signing. Their completed form is saved with their client record.
Getting started
- Ask Yuna to draft a form, choose a starting template, or create one yourself; review and publish the version you want clients to use.
- For automatic remote delivery, link the form to a service and choose when it should be sent.
- Keep a client email address or phone number on file for remote delivery. In-person kiosk signing uses the iPhone or iPad app instead.
Good to know
- Form templates are tools for your business, not legal advice; you are responsible for suitable wording and consent requirements.
- Remote form links and kiosk handoffs expire; start a new handoff or resend a link if the client does not finish in time.
- Only intake forms linked to a service can be required before its appointment is confirmed. Other forms are sent at the time you choose.
What this gives you
A traceable digital submission attached to the client history, with required appointment steps visible to the professional.
A PMU artist asks Yuna to draft an intake and consent form, checks the health questions and consent wording, and publishes it. A client who books online can complete it from a link; a walk-in can sign in kiosk mode on the artist's iPad during the visit. The artist reviews the answers before treatment.
